The worldwide shortage of organs and tissue for transplant has led to many ethical discussions involving restrictions concerning organ and tissue donation, including living donations and payment for donation. Efforts are being made to increase the donor pool; however, it is timely to ask whether moral decisions such as the rejection of directed donations are defensible. In this Clinical Conundrum, six specialists delve into the ethical and practical issues surrounding directed donation of human organs and tissues with particular reference to its implications in the field of ophthalmology.

Background: Despite favorable hemodynamic and neurohormonal effects, endothelin receptor antagonists have not improved outcomes in patients with heart failure (HF), possibly because they cause fluid retention.
Methods: In this randomized, double-blind, multicenter trial (SERENADE [Macitentan in Heart Failure With Preserved Ejection Fraction and Pulmonary Vascular Disease]), we evaluated the effects of an endothelin receptor antagonist, macitentan, in patients with HF, left ventricular ejection fraction ≥40%, and pulmonary vascular disease. After a 4-week placebo run-in (to ensure clinical stability), followed by a 5-week single-blind macitentan run-in, patients who did not exhibit fluid retention were randomized to macitentan or placebo.

In this review, the authors highlight recent advances in the care of liver transplantation patients important to anesthesiologists. The number of liver transplants continues to increase annually, and the challenges of managing complex patients require knowledge of the unique pathophysiology of end-stage liver disease. Regional anesthesia, specifically fascial plane blocks, allows for improved nonopioid pain control in recipients and potentially decreases length of stay.

Background: Maternal cytomegalovirus (CMV) infection at conception can cause congenital CMV (cCMV) infection and substantial morbidity. Although CMV screening is mandated for sperm donors, the risk of acquiring CMV from donor sperm is unknown. Experience with HIV may lead clinicians to expect that standard sperm-washing reduces CMV transmission risk for procedures including intrauterine insemination.
