Methotrexate used as a steroid-sparing agent in non-renal chronic Henoch-Schönlein purpura.

Clin Exp Rheumatol

Children's Hospital of Philadelphia, Division of Rheumatology, Department of Pediatrics, University of Pennsylvania School of Medicine, Philadelphia, Pennsylvania, USA.

Published: April 2004

Henoch-Schönlein purpura (HSP) is the most common form of systemic vasculitis in childhood and a small percentage of children develop a chronic form of the disease that often requires prolonged corticosteroid therapy. A detailed case description of a child with non-renal chronic HSP treated with methotrexate to allow for tapering of corticosteroids is presented. This represents the first report of methotrexate use in the treatment of chronic HSP.

Sterile inflammation has been increasingly recognized as a hallmark of non-infectious kidney diseases. Induction of pro-inflammatory cytokines in injured kidney tissue promotes infiltration of immune cells serving to clear cell debris and facilitate tissue repair. However, excessive or prolonged inflammatory response has been associated with immune-mediated tissue damage, nephron loss, and development of renal fibrosis.

Association of Oral Tobacco-Free Nicotine Delivery Product with Acute Renal Tubular Necrosis.

Medicina (Kaunas)

November 2024

Division of Pediatric Nephrology, Department of Pediatrics, University of Florida, Gainesville, FL 32610, USA.

Article Synopsis
  • The use of non-tobacco oral nicotine delivery products (ONDPs) is rising among U.S. adolescents, but their toxic effects, particularly on kidneys, are not well-researched.
  • A case study of a 19-year-old male kidney transplant patient revealed that after using ZYN pouches, he experienced severe acute tubular injury without rejection or other infectious causes.
  • Follow-up indicated that while his kidney function worsened initially, it improved over time, highlighting the need for more comprehensive studies on the renal impacts of ONDPs.

Association of uric acid with body adiposity and biochemical parameters in kidney transplant patients.

Clin Nutr ESPEN

December 2024

Laboratory of Nutrition, Exercise and Health (LaNES), School of Medicine, Federal University of Uberlandia (UFU), Uberlandia, Minas Gerais, Brazil. Electronic address:

Background: Obesity and various biochemical parameters, including triglycerides, cholesterol, glucose, C-reactive protein, and estimated glomerular filtration rate, have been linked to elevated uric acid (UA) levels in populations with normal kidney function due to decreased UA excretion and/or increased UA synthesis. However, it remains unclear whether all these factors exhibit similar associations with UA levels in clinical populations characterized by compromised renal function, such as kidney transplant patients (KTPs).

Objective: To evaluate whether serum UA levels are associated with body adiposity and biochemical parameters in KTPs.

We present a case of refractory cutaneous dermatomyositis (DM) in a 51-year-old Hispanic female which failed multiple treatments but found symptomatic relief with anifrolumab. Anifrolumab was the only treatment that was associated with significant improvement in the rash and pruritis of the patient and lowered her corticosteroid needs. To our knowledge, this is the only second case report that has shown success in treating refractory cutaneous symptoms of DM with anifrolumab after failing standard and multiple combinations of therapies.

Article Synopsis
  • Persistent acute kidney injury (pAKI) has a worse prognosis than transient acute kidney injury (AKI) in critically ill patients, but its impact and definitions are less understood in organ transplant recipients.
  • A systematic review of 25 studies involving 6,330 patients showed a wide variation in the incidence and definitions of pAKI among heart, lung, and liver transplant recipients.
  • pAKI is linked to higher rates of new chronic kidney disease, graft dysfunction, and long-term mortality, highlighting the need for standardized definitions in future research.
