We present a case of a patient with an intrathecal pump who experienced an unrecognized partial pocket fill, leading to an atypical opioid withdrawal characterized by akathisia. A 57-year-old female with multiple myeloma presented to an emergency department with new-onset akathisia requiring admission. Eight weeks prior, her intrathecal pump was refilled with morphine, bupivacaine, and ziconotide.

General anesthesia is the gold standard for breast cancer surgeries. Considering the nature of the surgery and its associated pain, various regional techniques are used as an adjunct to general anesthesia. Regional anesthesia as a sole anesthetic technique for breast cancer surgery is an upcoming technique - especially in high-risk patients considering the risk-benefit ratio, various regional blocks like pectoralis major block, pectoralis minor block, and erector spinae block - in which thoracic segmental spinal anesthesia is the recent one.

Opioid medications are commonly employed for perioperative and postoperative pain management. However, these medications can negatively impact the body's innate pain management system, specifically the action of beta-endorphins. By impairing the function of mu-opioid receptors and inhibiting the release of beta-endorphin, opioids may exacerbate and prolong postoperative pain.

Background: Patient safety is important in daily anesthesia practices, and providing deep anesthesia is difficult.
