Comparative estimation of the laparotomic and laparoscopic operative interventions efficacy was done in 75 patients with an acute biliary pancreatitis. In control group included 39 patients, operated using laparotomic access, in main group--36 patients, in which was done diagnostic and medical laparoscopic interventions. Application of laparoscopy have allowed to establish diagnosis, form of disease, presence of peritonitis and determinate medical tactics. In emergent order operated 8 (22.2%) patients with acute phlegmonous cholecystitis in combination with noninfected pancreatonecrosis. In 28 (77.8%) patients were performed medical laparoscopic interventions with endoscopic papillosphincterotomy.
