Siegrist streaks in giant cell arteritis.

A patient who presented with symptoms of giant cell arteritis was found to have a right ophthalmic artery occlusion. One month after initial evaluation, the peripheral retina demonstrated multiple linear bands of chorioretinal atrophy known as Siegrist streaks. Although most commonly described in the setting of acute hypertension, Siegrist streaks also occur in patients with giant cell arteritis.

Eclampsia is a severe hypertensive disease accompanied by tonic-clonic convulsions in the second half of pregnancy or during delivery in the absence of other aetiology. The association between eclampsia and serous retinal detachment is not common. We report a case of a 20-year-old primipara with eclampsia who developed bilateral exudative retinal detachment after delivery.

A 17 year-old Caucasian female presented with decreased vision, with no other symptoms reported at that time. Ophthalmological examination revealed typical signs of hypertensive choroidopathy, Elschnig spots, and Siegrist streaks. Further medical examination found high blood pressure (220/155mmHg), with a metastatic hormone-secreting adrenal carcinoma being the underlying cause of these alterations.

Untreated systemic hypertension is associated with retinopathy, optic neuropathy, and choroidopathy. Hypertensive choroidopathy is less common than retinal hemorrhages and infarcts seen with accelerated hypertension. The manifestations of choroidopathy include serous retinal detachment, Elschnig spots, and Siegrist streaks.
