Lung involvement in rheumatoid arthritis.

RA is a systemic autoimmune disorder primarily involving the joints. Extra-articular manifestations of RA often include lung involvement with heterogeneous clinical presentation and radiological findings. Autopsy studies reveal that the percentage of RA patients with pathological changes in the lung is significantly higher than that of patients with clinical manifestations. Lung alterations in RA may be primary or secondary to pharmacological treatments and may involve the alveoli, the interstitium, the airways and/or the pleura. These alterations may significantly impair lung function and some of them are potentially life-threatening. Thus, clinical examination and lung function testing should be performed in all patients with RA at the time of diagnosis and during follow-up. Those patients with clinical alterations and/or impaired lung function should undergo a complete radiological study.

Typical waveforms used for the simulation of pressure and volume-controlled ventilation in medical ventilators have been extensively studied in the literature. The majority of simulation studies reported employ the step pattern or ramp pattern to model the pressure and flow variations in pressure/volume-controlled ventilation. It was observed that the above waveforms tend to add to the discomfort level of patients due to the presence of jerks in derivatives of pressure/flow variations; the pressure/flow variation of air and oxygen mixture should be smooth so that the patient discomfort is kept at a minimal level.

Inflammatory diseases are often chronic and recurrent, and current treatments do not typically remove underlying disease drivers. T cells participate in a wide range of inflammatory diseases such as psoriasis, Crohn's disease, oesophagitis and multiple sclerosis, and clonally expanded antigen-specific T cells may contribute to disease chronicity and recurrence, in part by forming persistent pathogenic memory. Chronic rhinosinusitis and asthma are inflammatory airway diseases that often present as comorbidities.

Background: An initial decline in estimated glomerular filtration rate (eGFR) often leads to reluctance to continue life-saving therapies in patients with heart failure (HF).

Objectives: The goal of this study was to describe the association between initial decline in eGFR and subsequent clinical outcomes in patients randomized to placebo or finerenone.

Methods: In this prespecified analysis of FINEARTS-HF (Finerenone Trial to Investigate Efficacy and Safety Superior to Placebo in Patients with Heart Failure), we examined the association between initial decline in eGFR (≥15%) from randomization to 1 month and subsequent outcomes in patients assigned to finerenone or placebo.
