Background: During military operations or mass casualty situations, trauma surgeons are called on to place external fixation without the use of fluoroscopy. However, the safety and efficacy of this procedure has not been investigated.
Methods: Orthopedic surgeons, with varying experience, externally stabilized 20 fractured cadaver femora and tibiae. Using Behrens' safe zones, 194 half-pins were placed without radiographic assistance. Subsequently, fluoroscopy and dissections documented pin positions.
Results: Of the 194 pins, there were three (1.5%) neurovascular injuries (popliteal artery trifurcation). Forty-nine percent (95 of 194) of pins were overpenetrated (greater than two threads out the second cortex), with the mean distance to neurovascular structures of 10.2 mm. The mean overpenetration was +5.2 threads (13 mm).
Conclusion: When compared with radiography-assisted fixation, our results show the risk for neurovascular injury doubles with pin placement without fluoroscopic assistance. At less than 2%, this may be an acceptable rate in battlefield/mass casualty situations. This study also demonstrates that increased experience with standard fixation correlates to safer pin placement feel-placed fixation. In austere conditions, experienced surgeons can safely place external fixators without radiographic guidance.

Background And Objectives: The coexistence of complete carotico-clinoid bridge (CCB), an ossification between the anterior (ACP) and the middle clinoid (MCP), and an interclinoidal osseous bridge (ICB), between the ACP and the posterior clinoid (PCP), represents an uncommonly reported anatomic variant. If not adequately recognized, osseous bridges may complicate open or endoscopic surgery, along with the pneumatization of the ACP, especially when performing anterior or middle clinoidectomies.
Methods: According to Preferred Reporting Items for Systematic Reviews and Meta-Analyses for Scoping Reviews guidelines, a systematic scoping review was conducted up to June 5, 2023.

Anterior cruciate ligament reconstruction (ACLR), despite its effectiveness in restoring knee stability and function, can have associated morbidity. The most frequent complications are technical errors, which have been described during graft harvesting, tunnel placement or graft fixation. The most serious complications are neurovascular injuries, arthrofibrosis and infections.

The first cervical vertebra (C1) is atypical in shape and bears a complex relationship with important neurovascular structures such as the vertebral artery and cervical spinal cord which are at risk of injury during misplaced screw fixation of C1. Placement of screws into the lateral mass of C1 vertebra is performed for stabilization of the craniovertebral junction. The objective of this study was to describe ideal screw dimensions, precise entry points, safe bony corridors, and ideal trajectories for placement of lateral mass screws in the Emirati population.

Over the last two decades the development of small probes has enabled percutaneous use of cryotherapy. Cryotherapy, also known as cryoablation, enables the treatment of much larger lesions than other thermal ablation techniques, particularly when using multiple evenly spaced probes. Using rapid cooling to as low as -200 degrees Celsius (at the tip of the probe), reliable, and predictable necrosis can be induced.

Ischemic stroke and vascular cognitive impairment, caused by a sudden arterial occlusion or more subtle but protracted vascular insufficiency, respectively, are leading causes of morbidity and mortality worldwide with limited therapeutic options. Innate and adaptive immunity have long been implicated in neurovascular injury, but recent advances in methodology and new experimental approaches have shed new light on their contributions. A previously unappreciated dynamic interplay of brain-resident, meningeal, and systemic immune cells with the ischemic brain and its vasculature has emerged, and new insights into the frequent overlap between vascular and Alzheimer pathology have been provided.
