Objectives: To compare the benefits of transurethral resection of the prostate in urodynamically obstructed versus selected urodynamically unobstructed or equivocal men with severe lower urinary tract symptoms associated with benign prostatic enlargement.

Methods: In this case series study, men with lower urinary tract symptoms were selected if they met the study criteria and underwent the tests recommended by the International Scientific Committee on Benign Prostatic Hyperplasia. They also underwent urodynamic investigations. Men were included when transurethral resection of the prostate was selected as the treatment modality. Of the 132 included subjects, 93 could be re-evaluated 6 months after surgery.

Results: Of the 93 re-evaluated men, 59 were obstructed and 34 were unobstructed or equivocal. Both groups were similar with respect to age, symptoms, bother, benign prostatic hyperplasia-impact index, and quality of life. The quantified reductions in symptoms and bother in the unobstructed and equivocal men were about 70% of those reductions in the obstructed men. In the equivocal men, and even in the unobstructed men, a significant reduction with 40% of the urethral resistance occurred.

Conclusions: Transurethral resection of the prostate may be a good treatment alternative for unobstructed or equivocal men with severe lower urinary tract symptoms associated with prostatic enlargement, who opt for resection or who do not respond to or do not tolerate medical therapy.

Omega-3 fatty acids and sleep: recent advances in understanding effects and mechanisms.

Curr Opin Clin Nutr Metab Care

December 2024

Brain, Performance and Nutrition Research Centre, Faculty of Health and Life Sciences, Northumbria University, UK.

Purpose Of Review: Sleep disturbance and low dietary intake of omega-3 fatty acids are common in modern society. Previous research shows that omega-3 fatty acids play an integral role in the regulation of sleep processes, but efficacy data in adults have been equivocal to date. This review examines recent studies highlighting the effects of omega-3 fatty acids on sleep quality and circadian processes.

Background: Multiparametric magnetic resonance imaging (mpMRI) is now the standard of care to guide prostate biopsies during workups and assessment of men with suspected prostate cancer (PCa). In addition to intraprostatic lesion detection, MRI usually covers the bony pelvis and pelvic lymph nodes, two of the commonest sites for metastatic disease. Subsequent staging has traditionally been based on further scanning using a combination of computed tomography (CT) and bone scintigraphy (BS), and more recently, positron emission tomography (PET) scanning with prostate-specific membrane antigen (PSMA) ligand.

Cutaneous ureterostomy (CU) is the most simple and safe method of all permanent urinary diversions, but is associated with a risk for stomal obstruction. It is important to appropriately manage hydronephrosis associated with CU. We evaluated the occurrence of stomal obstruction after CU by 99m Tcmercaptoacetyltriglycine (MAG3) diuretic renography three months after surgery.

People living with HIV (PLWH) and people who use drugs are vulnerable populations who may face barriers to accessing health services and may have irregularities in immune function. People with undiagnosed HIV infection may be particularly likely to have compromised immune function. However, research about whether/how HIV status is related to COVID-19-related health outcomes has been equivocal, and research on the predictors of COVID-19-related health service access/uptake has been limited in Sub-Saharan African settings.

Background: To evaluate the effectiveness of a deep learning denoising approach to accelerate diffusion-weighted imaging (DWI) and thus improve diagnostic accuracy and image quality in restaging rectal MRI following total neoadjuvant therapy (TNT).

Methods: This retrospective single-center study included patients with locally advanced rectal cancer who underwent restaging rectal MRI between December 30, 2021, and June 1, 2022, following TNT. A convolutional neural network trained with DWI data was employed to denoise accelerated DWI acquisitions (i.
