HIV infection frequently affects multiple systems, with hematological manifestations being the most prevalent. In some cases, cryptococcosis serves as the initial manifestation and a cause of infection involving HIV-positive patients. This case report describes a patient with thrombocytopenia who incidentally discovered infiltrating the bone marrow upon bone marrow smear examination, highlighting that examining bone marrow is essential in diagnosing pancytopenia resulting from opportunistic fungal infections like cryptococcosis, especially in individuals with compromised immune systems.

Invasive fungal diseases are an important public health concern due to an increase in the at-risk population and high mortality associated with these infections. Managing invasive fungal infections poses a significant challenge given the limited antifungal options and the emergence of resistance in key fungal pathogens. Background: Cryptococcosis is an opportunistic fungal infection in immunocompromised patients. The major species include Cryptococcus grubii, Cryptococcus neoformans, and rarely, Cryptococcus gattii. Here we present a disseminated Cryptococcus gattii infection in a patient with elevated granulocyte-macrophage colony-stimulating-factor autoantibody which was successfully treated with antifungal therapy.
