Endocardial defibrillation lead extraction: an unusual case of entrapment.

Spurious discharges due to late insulation break in an IS-1 pacing/sensing connector prompted ICD lead removal in 65-year-old man. The tip of the lead was easily freed and pulled back into the SVC by the superior approach. After that, the lead became trapped. The distal part of the lead was caught and easily withdrawn by inferior approach. Superior venous angiography showed extravascular location of the entrapped part of the lead due to the unintentional percutaneous puncture of the innominate vein after piercing the subclavian vein. It may be desirable to use contrast venography before intervention of extraction to ensure venous patency and lead location.

Falling Rates of Public Orthopaedic Surgery Reimbursements and Utilization, 2016 to 2024.

Background: Declining reimbursement rates can lead to decreased access and utilization of common orthopaedic surgeries for patients on Medicare, which is a particularly vulnerable population for musculoskeletal injuries.

Methods: Using the Centers for Medicare & Medicaid Services Physician Fee Schedule Look-Up Tool from 2016 to 2024 and utilization data for Medicare and part B beneficiaries from 2016 to 2022, we analyzed reimbursement and utilization trends. Simple linear regressions were executed to measure the annual trends, and Wilcoxon matched-pairs signed rank test were used to analyze the statistical significance of price and utilization changes.

In this study, we report the modification of a monolithic γ-aluminum oxy-hydroxide (γ-AlOOH) aerogel with cellulose nanofibers (CNFs) using the sol-gel method via supercritical drying. The optimized 2% CNF (w/w) results in a monolithic CNF-γ-AlOOH that is amorphous in nature, along with C-C and C-O-C functional groups. Transmission electron microscopy (TEM) images of the as-synthesized CNF-γ-AlOOH showed CNF embedded in the γ-AlOOH aerogel.
