Objective: To study the effect of vascularized fibular graft on large defects of long bones and the monitoring method for the vascular status of the grafted fibula.

Methods: From March 1988 to February 1998, 30 patients with long bone defects over 6 cm in length received vascularized fibular graft including a monitoring island flap in our department to monitor the blood circulation of the fibulae.

Results: Monitoring island abnormalities were indicated in 6 flaps, which accurately gave the alarm of the circulation crisis of the fibular graft. Surgical exploration was performed timely and the blood supply was recovered instantaneously. All the bone defects were healed at 6 months postoperatively. After 4 years of follow-up, all the grafted fibulae were thickened and were just like tibiae.

Conclusions: A monitoring island flap is a satisfactory method for repairing large defects of the long bones and a reliable method for assessing the vascular status of the grafted fibulae.

We examined whether the glucose levels and awareness of individuals without diabetes changed after using a sensor-based intermittently scanned continuous glucose monitoring (isCGM) system in their daily lives. Japanese individuals without a diabetes diagnosis wore the isCGM system while maintaining a normal lifestyle during the baseline period. A certified diabetes educator coached them on how to improve their lifestyle based on information from sensor data, food journals, and body composition.

On 6 November 2010, a mass stranding event (MSE) occurred on Rutland Island, Co Donegal involving 33 long-finned pilot whales (Globicephala melas). While the cause of the stranding remains unknown, the event enabled the collection of a large number of tissue samples for opportunistic quantification of Persistent Organochlorine Pollutants (POP) and from across a spectrum of age-classes, maturity, and sexes. The concentrations of 24 Polychlorinated Biphenyls (PCBs), 15 Organochlorine Compounds (OCs), 5 Brominated Flame Retardants (BFRs) and 17 Polychlorinated-dibenzo-dioxins/furans (PCDD/Fs) were investigated in blubber samples via lipid extraction and subsequent analysis by Gas Chromatography-Mass Spectrometry (GC-MS).

Importance: Despite growing criticism of alcohol consumption due to its overall health risks, it remains unknown how changes in alcohol consumption, particularly cessation, affect lipid profiles outside of intense interventions.

Objective: To clarify the association of alcohol initiation and cessation with subsequent changes in low-density lipoprotein cholesterol (LDL-C) and high-density lipoprotein cholesterol (HDL-C).

Design, Setting, And Participants: This cohort study included individuals undergoing annual checkups at a center for preventive medicine in Tokyo, Japan, from October 2012 to October 2022.

Background: Rare and nutritionally complex diseases, like chronic intestinal failure (CIF), are not encountered regularly by clinicians. Specialized centers with experience in intestinal failure are relatively sparse, leading many patients to rely on local clinicians. Previously published reports demonstrated lack of knowledge in intestinal failure among gastroenterologists in the United States, in Europe, and in the United Kingdom.
