Recently, drug polyuria has found wide application in diagnosis and treatment: to detect latent urinary insufficiency, to assess functional performance of the contralateral (unaffected) kidney in nephrolithiasis and calculous pyelonephritis, to stimulate elimination of fragments of the disintegrated concrement, to check up suture impermeability after surgical repair of the urinary tracts, to outline afunctional zone in hydronephrosis. Clinical experience gives grounds for use of saluretics at certain stages of pyelolithotomy for mobilization of intrarenal pelvis and its dissection, removal of the calculus and subsequent suturing of the intrarenal pelvis. Therapeutic polyuria can also facilitate ureterolithotomy.

Blockade of the actions of the renin-angiotensin-aldosterone axis by inhibition of the angiotensin-converting enzyme with captopril is a new tool in the field of hypertension. As treatment with captopril makes pressure control volume-dependent, the addition of a saluretic is considered the best choice if an additional hypotensive agent is needed. The reason for and experience of the use of captopril in combination with saluretics for treatment of severe hypertension is reviewed.

It is reported on the clinical experiences with the new diuretic azosemid in 22 patients with oedemas of various genesis. Azosemid displays the characteristics of a very effective and well tolerated saluretic. After intensive treatment azosemid exerts similar side effects on the electrolyte (hyponatraemia, hypokalaemia, hypochloraemia) and purine metabolism (hyperuricaemia) as furosemide.

Under general practice conditions 18 patients with congestive heart failure which did not respond satisfactorily to digitalization and saluretic therapy were treated with Osyrol 100/Lasix for a period of 3 months while retaining the digitalis medication. The application gave good to satisfactory results in over 90% of the cases. This very good result in patients with previously unsatisfactory response underlines the high therapeutic efficacy of this preparation.
