Soft tissue deficiency around dental implants can negatively impact outcomes in terms of esthetics and long-term stability. While autogenous connective tissue grafting is still considered the gold standard treatment, alternative approaches are being proposed primarily to enhance patient comfort and avoid invasive procedures such as two-sites surgeries using xenogeneic collagen matrices. Despite the advantages, the quality of the regenerated tissues remains unpredictable and, in many cases, questionable, highlighting the need for alternative and innovative approaches.

Anterior disc displacement without reduction (ADDwoR) is one of the most common types temporomandibular disorders (TMD), which is often characterized by joint pain, abnormal joint sounds, and limited mouth opening. Disc repositioning has been described as an effective method to reduce joint pain and improve range of motion. Yang's arthroscopic disc repositioning and suturing surgery has been reported to be the most stable technique with 95%-98% of success rate.

However, it is essential to note that the LPM's structure is not a constant feature, as the number of bundles and their topography can vary. The reported complications after arthrocentesis of the temporomandibular joint are rare, which makes it to an outpatient and safe oral and maxillofacial surgical procedure. A penetration into the middle cranial fossa is an extremely rare but serious complication for the conventional needle arthrocentesis. The simple technical modification described herein could help to easily retain the puncture depth via an endodontic silicone stopper to avoid this rare complication.
