Objectives: Intensive care unit (ICU) clinicians stop antibiotics more often, with a negative infection: point-of-care test (PCR-POCT). Simulated cases of diagnostic uncertainty regarding infection resolution led clinicians to choose options such as procalcitonin (PCT) and/or PCR-POCTs +/- de-escalation to aid stop decisions. We hypothesised that a direct infection indicator, PCR-POCT, would influence stop judgements more than indirect PCT.

The aim of our study was to analyze the potential survival benefit associated with HSCT according to clinico-biological scores which incorporate molecular data (MIPSS70 and MIPSS70+V2) to facilitate decision-making in this context. One transplant (n=241) and one non-transplant cohorts (n=239) were used to test the hypothesis that PMF patients with higher risk molecular score benefit from HSCT. A weighted propensity score was applied to balance confounding factors with the transplanted cohort as reference.

A regression predictive model for QuantiFERON-TB Gold Plus indeterminate results in immunosuppressed patients.

SAGE Open Med

September 2024

Research Laboratory in Immunology of Renal Transplantation and Immunopathology (LR03SP01), Charles Nicolle Hospital, Tunis El Manar University, Tunis, Tunisia.

Background: Screening for latent tuberculosis infection using Interferon-Gamma Release Assays is a routine procedure prior to the initiation of anti-tumor necrosis factor (TNF) biotherapy or immunosuppressive therapy. However, indeterminate results are relatively frequent and are an obstacle to treatment initiation.

Aim: The aim of this cross-sectional study was to estimate the frequency of indeterminate QuantiFERON-TB Gold Plus test results in Tunisian patients, and to analyze the potential clinico-biological risk factors associated with these indeterminate results.

[Implementation of edoxaban anti-Xa activity assay in a hospital laboratory and evaluation of its analytical performance].

Ann Biol Clin (Paris)

September 2024

Service d'Hématologie Biologique, Assistance Publique Hôpitaux de Paris-Centre (AP-HP.CUP), F-75015 Paris, France, Université Paris Cité, Innovative Therapies in Haemostasis, INSERM, 75006 Paris, France, INNOVTE, F-CRIN, Saint-Étienne, France.

Article Synopsis
  • Edoxaban is a direct oral anticoagulant not available in France, making it important to understand its pharmacology and lab testing, especially with high tourist traffic.
  • The study focused on measuring the anti-Xa activity of edoxaban using a specific chromogenic method, detailing pre-analytical and analytical processes for accurate therapeutic guidance.
  • Results showed the measurement method to be reliable and straightforward, suggesting that even though it's seldom used clinically, having the ability to measure edoxaban levels in French hospitals is important due to its availability in nearby countries.

Clinico-biological factors predicting the benefit of the LV5FU2 maintenance strategy as a first-line therapy in patients with metastatic pancreatic cancer.

Oncologist

September 2024

Department of Gastroenterology, University Hospital Pontchaillou, Rennes 1 University, INSERM U1242 "Chemistry Oncogenesis Stress Signalling," Rennes, France.

Article Synopsis
  • The study investigates predictive markers for the benefits of LV5FU2 maintenance therapy after first-line FOLFIRINOX treatment in patients with metastatic pancreatic cancer.
  • It compares two patient groups from the PRODIGE-35 trial: one receiving 12 cycles of FOLFIRINOX, and another receiving 8 cycles followed by LV5FU2 maintenance.
  • Results indicate that certain factors, like age and metastatic sites, affect patient outcomes, suggesting that LV5FU2 maintenance is generally beneficial except for some specific cases.
