Background And Objectives: Congenital thumb hypoplasia is a rare deformity of upper extremity. The incidence for thumb hypoplasia grade II-V is 1:10,000 newborns per year in Latvia. A technique for extensor indicis proprius (EIP) tendon transfer with subperiosteal fixation was developed and used for thumb hypoplasia grades II and IIIa.

Background: Pollicization of the index finger is a well-established treatment in type IIIB to type V hypoplastic thumbs. However, there is a lack of quantitative outcome studies, and little is known about the outcome differences between patients with mild and severe longitudinal radial deficiencies. Therefore, the aim of this study was to quantitatively assess and compare outcome after pollicization in severe and mild longitudinal radial deficiency and compare outcome to healthy controls.

A 27-year-old otherwise healthy patient was diagnosed with a primitive neuroectodermal tumor of the thumb metacarpal bone of the left hand. Based on a common chromosomal translocation this tumor shows a close relationship to Ewing's sarcoma. Its occurrence in the extremities is uncommon and involvement of the hand is extremely rare.

Purpose/background: A reconstructed thumb has to fulfil the function of opposition, pinch-grip, stability, sufficient length, sufficient sensibility, and painlessness. Next to pollicisation, these requirements can only be met satisfactory by the microvascular free great toe-to-hand transplantation. The goal of a thumb reconstruction is a compromise between an optimal functional and aesthetic outcome and a minimal morbidity of the donor site.

Reconstruction of the thumb after traumatic mutilation constitutes a delicate indication in order to select the technique most adapted to the patient's functional needs and esthetic desires. Pollicizations of healthy fingers are now used less frequently, except in the clinical context of very proximal amputations, but amputations of mutilated fingers remain a frequent indication. Progressive lengthening was associated with a high complication rate, although these complications have remained minimal.
