Purpose: To evaluate the use of a superior mesenteric artery (SMA) approach to embolize type II endoleaks arising from the inferior mesenteric artery (IMA).

Technique: When reperfusion of the aneurysmal sac via the SMA occurs through the IMA, as shown by computed tomography (CT) and angiography, the IMA origin can be accessed via the marginal artery or the anastomosis of Riolan. The SMA is catheterized with a 5-F catheter, and a coaxial catheter is advanced to the leak to deliver 2- to 8-mm-diameter minicoils to embolize the IMA origin and entire aneurysmal sac.

Purpose: Evaluation of clinical usability and effectivity of newly developed, mechanically applicable minicoils attached to a wire for super-selective embolisation.

Material And Methods: The new embolisation coils have been used in 16 patients aged between 28 and 90 years for the following indications: 4 haemorrhages in cases of advanced carcinoma of the cervix, one false aneurysm, 7 traumatic lesions and 4 arteriovenous fistulas. The minicoils are made of platinum and are attached to a guide wire with a connecting hook.

The authors' experience in the management of postoperative vaginal hemorrhage from 1975 to 1980 was reviewed. Recently, success has been achieved using angiographic arterial embolization with the Gianturco minicoil. The results of embolization are compared with those achieved through other more conventional methods.
