Osteoarthritis of the knee joint is a public health concern with considerable social impacts and related-costs. Among the treatments available, several benefits of bathing in natural mineral water have been proposed: (1) to identify possible health benefits (in terms of effects on dimensions of pain, functionality, emotional and social aspects, and quality of life) of a 3-week balneotherapy intervention in patients with knee osteoarthritis; (2) to assess the clinical relevance of any benefits detected; and (3) to determine if these effects persist. Participants of this randomized controlled trial (RCT) were 120 patients randomly assigned to (1) an experimental group (3 weeks of balneotherapy consisting of daily whirlpool baths, hydrokinesitherapy sessions, and knee shower/massages) or (2) control group in which no form of treatment apart from their usual analgesia medication was given.

Recent mechanistic evidence demonstrates that spa-based therapy (or, as we propose, crenotherapy from the Greek word kapparhoepsilonnueta, spring fountain) is indeed based on solid scientific data. This mini-review highlights the latest insights into the mechanisms of crenotherapy derived from in vitro experiments, studies on animal models, and carefully designed clinical trials. Although more basic and clinical data are still needed, crenotherapy is coming of age as a modern, scientifically sound therapy.

In preliminary studies, crenotherapy has emerged as a possible effective treatment for general anxiety disorder. We have compared crenotherapy to Paroxetine in a randomized multicentric control trial for a period of 8 weeks. 237 patients who met the diagnosis criteria of generalized anxiety disorder (DSM-IV) were recruited, 117 patients were randomly assigned to Crenotherapy and 120 to Paroxetine.

Several aspects comprise the medical treatment of chronic venous insufficiency: adoption of daily routines favouring mechanisms for return of venous blood; wearing of elastic stocking to reduce superficial venous hypertension, the basis of medical treatment; use of venotonic agents as adjuvant therapy; association of crenotherapy; and finally the possibility of sclerotherapy when such treatment can correct the dysfunction of the superficial venous network. Various therapeutic strategies should be considered to manage not only the functional and/or clinical signs of chronic venous insufficiency, but also the complications which may occur (varicose thrombosis and haemorrhage, or deep venous thrombosis). Lastly, prevention is based not only on respect of daily hygienic measures but above all on early treatment that is adapted to any deep venous thrombosis.
