We report the case of a woman who had been operated upon for a recurrent meningioma of the falx cerebri which, 22 years after it was discovered, produced pulmonary metastases. Metastases from meningiomas are extremely rare; they affect the lungs in 60% of the cases or, less frequently, the liver or lymph nodes. The factors predictive of secondary dissemination are the site of the tumour, the quality of excision, the histological type (haemangiopericytic meningioma) and the presence of cell necrosis, hypercellularity and strong mitotic activity.

Rosai-Dorfman-Destombes disease (RDD) is a rare histiocytic proliferative disorder, with primary scalp and intracranial involvement being particularly uncommon. The imaging features and clinical manifestations of RDD often overlap with other intracranial lesions, such as meningiomas, leading to misdiagnosis. This study presents a case of primary scalp and intracranial RDD, with a comprehensive analysis of its imaging, pathological, and intraoperative findings, alongside a review of the literature on central nervous system (CNS) RDD and its diagnostic and therapeutic advancements.

Objective: The resection of parasagittal and falx meningiomas in the middle third superior sagittal sinus (SSS) is associated with a high risk of postoperative motor deficits. This study discusses the risk factors affecting postoperative motor decline and recovery of motor function after follow-up.
Methods: The results of a single-institution retrospective cohort study of parasagittal and falx meningioma resection in the middle third between 2016 and 2023 were reviewed, and parameters were screened as potential predictors.

Background And Objectives: The medial parietooccipital and peritrigonal areas present challenges for neurosurgical procedures. Adjacency to the eloquent cortex-postcentral gyrus and cuneus, as well as crucial white matter tracts, such as optic radiation-makes the surgical approaches difficult. This study aims to describe the surgical technique and outcomes of treating lesions using the contralateral approach.

Calcified chronic subdural hematoma (CCSDH) is a rare condition characterized by the accumulation of calcified blood between the dura mater and arachnoid membrane, typically following remote trauma. These lesions often present as space-occupying, extra-axial masses over the cerebral convexity and can mimic extra-axial tumors, such as calcified meningiomas. A 73-year-old male with a history of prostate cancer, hypertension, and hyperlipidemia presented with vision changes and mild papilledema.

Purpose: Digital 3D exoscopes are promising tools for microneurosurgery. The results of exoscope-assisted resection of intracranial meningiomas have only been addressed in few case reports. We retrospectively compared the results of exoscope and microscope-assisted surgery of falx and parasagittal meningiomas.
