Antimicrobial resistance and impaired bone regeneration are the great challenges in treating infected bone defects. Its recurrent and resistant nature, high incidence rate, long-term hospitalization, and high medical costs have driven the efforts of the scientific community to develop new therapies to improve the situation. Considering the complex microenvironment and persistent mechanisms mediated by resistant bacteria, it is crucial to develop an implant with enhanced osseointegration and sustained and effective infection clearance effects.

Background: The periosteum consists of an outer fibrous layer and an inner cellular layer, where bone cells reside. Hence, it has been suggested that applying periosteum to a periodontal defect may help new bone formation. The purpose of this case study is to present the clinical and radiographic outcomes of a vestibular regenerative approach and the application of a connective tissue graft (CTG) with periosteum to improve the periodontal prognosis of a pathologically migrated hopeless tooth with an endo-periodontal lesion (EPL).

The use of bioresorbable compositions has been considered a promising therapeutic approach for treating compromised bone tissues. Gellan gum (GG) is a predominant polysaccharide recognized for its exceptional biocompatibility and biodegradability, facile bio-fabrication, and customizable mechanical attributes, rendering it well-suited for developing versatile bone scaffolds. On the other hand, MXene nanosheets have been declared a representational filler to augment the osteogenic effect and amend the mechanical properties of the polymeric biomaterials.

Exosomes are natural membrane-enclosed nanovesicles (30-150 nm) involved in cell-cell communication. Recently, they have garnered considerable interest as nanocarriers for the controlled transfer of therapeutic agents to cells.
