This paper presents a case of a 77-year-old patient diagnosed with type 2 cardio-renal syndrome, who has undergone a Transcatheter Aortic Valve Implantation surgery due to aortic stenosis associated with permanent atrial fibrillation and type 2 diabetes. This patient, despite their multi-morbidity, undertook family travels to Egypt. Despite disease symptoms and plethora of medication, the patient did not seek medical advice on preventative measures or potential health risks prior to the departure.

Importance: It is not well understood if and how various social and environmental determinants of health (SEDoH) are associated with mortality rates related to cardio-kidney-metabolic syndrome (CKM) across the US.

Objective: To study the magnitude of the association strength of SEDoH with CKM-related mortality at the county level across the US.

Introduction: Peritoneal ultrafiltration (PUF) has been proposed as an additional therapeutic option for refractory congestive heart failure (RCHF) patients. Despite promising observational studies and/or case report results, limited clinical trial data exist, and so far, PUF solutions remain only indicated for chronic kidney diseases. In this article, we described a multicenter, randomized, controlled, unblinded, adaptive design clinical trial, about to start, investigating the effects of PolyCore™, an innovative PUF solution, in the treatment of RCHF patients.
