We describe the electrical signs due to the positional changes of the heart and those related to hypertrophy and dilatation of the right heart chambers in subjects with chronic cor pulmonale. The positional changes mainly reflect the vertical position of the heart, which is almost constantly associated with some degree of clockwise rotation, because of the descent of the diaphragm and increase of lung volume owing to chronic emphysema. The electrovectorcardiographic alterations observed in right ventricular hypertrophy owing to permanent pulmonary arterial hypertension differ depending on the type of hypertrophy: global or segmentary.

Myocardial function was studied in 131 patients with infectious-allergic bronchial asthma. The chronic cor pulmonale formation was shown to be a complex process including hypertrophy not only of the right but also of the left ventricle caused by raised pressure both in the lesser and greater circulation. Functional changes prevailed at the initial stages of the development of disease, but with progression of disease, especially in a severe course, not only functional but also irreversible organic myocardial changes were revealed.

Fifty-three consecutive patients, mean age 63 years, undergoing either peripheral vascular reconstructive surgery (n = 40) or lobectomy for bronchial carcinoma (n = 13) were examined pre- and postoperatively with conventional electrocardiogram (ECG), vectorcardiogram (VCG) and estimation of serum levels of aspartate aminotransferase, alanine aminotransferase, total lactic dehydrogenase and the heat-stable fraction of lactic dehydrogenase for the diagnosis of per/postoperative myocardial infarction (MI). Six patients (11%) developed signs of acute MI. In 2 patients whose ECG showed only unspecific changes, the VCG was decisive for the diagnosis.

The article discusses the possibilities of differential diagnosis of supraventricular arrhythmians by means of carotid zone stimulation, atrial VCG and ECG recorded from right-bronchial leads. Atrial VCG can reveal the character of the ectopic P-wave and thus helps in the diagnosis of arrhythmias with conspicuous atrial activity. ECG recorded from bronchial leade makes it possible to obtain a large atrial wave and determine elements of decisive importances in the diagnosis of many rhythm disorders: junctional and combined contractions, various types of ectopic P-waves, the character of antero- and retrograde conduction.
