We report a rare case of adenosquamous carcinoma of the gall bladder (GB) causing portal vein tumor thrombus. A 40-year-old gentleman presented with acute-onset right upper abdominal pain. Ultrasonography revealed multiple calculi in the GB with wall thickening, suggesting acute cholecystitis.

Thick-walled Gallbladder: A Pragmatic Management Approach.

Euroasian J Hepatogastroenterol

December 2024

Department of General and Minimal Invasive Surgery, Sher-I-Kashmir Institute of Medical Sciences Soura, Srinagar, Jammu and Kashmir, India.

Introduction: Thick-walled gallbladder (TWGB) is a common yet non-specific radiological finding associated with a wide range of gallbladder pathologies, including acute and chronic inflammation, infection, and malignancy. Among the inflammatory causes, xanthogranulomatous cholecystitis (XGC) is a rare but significant condition that often mimics gallbladder carcinoma. This paper presents a pragmatic approach to the diagnosis and management of TWGB, focusing on the complexities posed by XGC.

Radiomics is a method that extracts many features from medical images using various algorithms. Medical nomograms are graphical representations of statistical predictive models that produce a likelihood of a clinical event for a specific individual based on biological and clinical data. The radiomic nomogram was first introduced in 2016 to study the integration of specific radiomic characteristics with clinically significant risk factors for patients with colorectal cancer lymph node metastases.

Disulfiram/Cu(DSF/Cu) has a known pharmacokinetic and safety profile, exerting a strong antitumor effect. Oral tyrosine kinase inhibitors including lenvatinib are approved as first-line therapy for treating advanced unresectable hepatocellular carcinoma (HCC). These patients still have limited survival due to drug resistance.

Article Synopsis
  • Carcinoma of the gall bladder (CAGB) has a low survival rate, but analyzing bile samples can help identify molecular indicators for early detection of the disease.
  • A study on 87 bile samples found specific proteomic and metabolomic signatures associated with CAGB that correlated with increased inflammation and altered energy pathways compared to healthy controls.
  • The top four metabolites showed over 99% accuracy and sensitivity for CAGB detection in further validation tests, suggesting that bile analysis could lead to improved early diagnosis methods.
