We report a case of primitive pituitary abscess (PA) in a teenage girl who was admitted on three occasions for acute meningitis. A lumbar puncture revealed aseptic meningitis and the brain MRI showed a well-defined intrasellar space-occupying lesion with suprasellar extension suggestive of macroadenoma. The patient remained free of symptoms between episodes of meningitis however she developed a diabetes insipidus.

Pituitary abscesses are rare entities that may occur in a previously healthy gland or in the setting of a pituitary tumor. Only eleven cases of abscesses associated with craniopharyngioma have been reported in the literature. The etiology is unknown and the diagnosis is difficult because there are no specific clinical or radiologic features that allow us to suspect the synchrony of both entities before surgery.

Fever of unknown origin (FUO) presents a diagnostic challenge, requiring detailed evaluation of infectious, autoimmune, inflammatory, and neoplastic causes. We report the case of a 28-year-old female patient with a history of polycystic ovary syndrome, pituitary microadenoma, and prior thromboembolic events, who presented with two months of persistent fever, abdominal pain, and diarrhea. The patient recently traveled to Europe and reported freshwater swimming and walking barefoot on beaches.

Intracranial abscesses are a common cause of morbidity and mortality in neurologic free-ranging white-tailed deer (). These infections are typically attributed to and occur most often in adult males with concurrent antler and skull infections. A subset of intracranial abscesses in white-tailed deer are localized to the pituitary gland, with previously described reports in five white-tailed deer.

Nocardia species are aerobic, Gram-positive actinomycetes often causing opportunistic infections, particularly in immunocompromised hosts. Nocardia farcinica is notable for disseminated nocardiosis (DN), frequently involving the lungs and central nervous system (CNS). Cushing's syndrome (CS), characterized by hypercortisolemia, further predisposes patients to severe infections.
