The diagnosis and management of complex neurological conditions such as New-Onset Refractory Status Epilepticus (NORSE) and central nervous system (CNS) infections caused by non-tuberculous mycobacteria (NTM) pose substantial difficulties in intensive care units (ICUs). This article combines a case report and a literature review that explores the diagnostic dilemmas and therapeutic strategies for these critical conditions. We report the case of an 83-year-old female with chronic granulomatous meningitis secondary to NTM, presenting a challenging diagnostic and complex management complexity typical of such a rare disease through a period time of five years.

Article Synopsis
  • * The case report outlines four instances of extrapulmonary NTM disease in immunocompetent patients, with varied clinical outcomes ranging from meningitis and brain abscesses to cervical lymphadenopathy, highlighting misdiagnoses with tuberculosis.
  • * The findings emphasize the importance of awareness among clinicians regarding NTM disease, as early diagnosis and appropriate treatment are crucial for improving patient outcomes, especially for those with more severe infections.

Nontuberculous mycobacteria associated intracranial infection is a rare disease that mainly occurs in HIV-infected patients. The disease has a poor prognosis. The authors report a case of non-tuberculous mycobacterial meningoencephalitis in a non-AIDS patient, but long history of poorly controlled type 2 diabetes mellitus.

Article Synopsis
  • Meningitis is an inflammation of the protective layers around the brain and spinal cord, caused by infections or non-infectious factors like cancer or injuries.
  • The review compares tuberculous meningitis (TBM) and non-tuberculous meningitis (NTM), focusing on their clinical features to improve diagnosis.
  • Research on the roles of oxidative stress and ferroptosis in TBM suggests new treatment options aimed at reducing damage in the central nervous system.

The value of gene Xpert MTB / RIF, ADA, TB-DNA in the early diagnosis of TB meningitis.

Cell Mol Biol (Noisy-le-grand)

June 2023

Department of Tuberculous, Xi'an Chest Hospital, Xi'an, Shaanxi710100, China.

This study was to explore the expression and correlation between gene Xpert MTB / RIF, ADA, and TB-DNA in TB meningitis. For this purpose, we selected 102 patients in the TB meningitis progression diagnosed and treated in our hospital from January 2019 to December 2020, and another 100 patients in the non-TB meningitis group were selected for the control experiment. Two sets of CSF samples were taken to analyze the gene Xpert MTB / RIF positive rate and the correlation between the expression and the progression of tuberculous meningitis by testing the levels of ADA and TB-DNA in the patient body using an automatic biochemical analyzer.
