Drug-Resistant Bone, Joint and Spine Tuberculosis: Evolution of Diagnosis and Treatment.

Indian J Orthop

June 2024

University College of Medical Sciences, University of Delhi, A10 Part B, Ashok Nagar, Ghaziabad, 201002 India.

Background: Drug resistant (DR) osteoarticular TB (OATB) is a challenge in view of it being deep seated lesion and paucibacillary disease. Case definition, investigation protocol, treatment of proven DR and those cases where DR could not be demonstrated lacks clarity and evidence. Hence, a series of studies were conducted to develop an algorithm to investigate and treat therapeutically refractory disease (TRD) or presumptive drug resistance (PDR) cases of OATB.

  • An expert group utilized a modified Delphi method to reach a consensus on the recommendations, which include screening practices and diagnostic criteria for ABPA and ABPM in both adults and children.
  • Key recommendations include routine screening for fungal sensitization in specific patient groups, clear diagnostic criteria for ABPA and ABPM, and suggested treatments such as oral prednisolone or itraconazole for acute cases, while noting that asymptomatic ABPA patients do not require treatment.

Posterior reversible encephalopathy syndrome (PRES) is a clinico-radiological entity occurring during several conditions and more frequently in those associated with severe hypertension. The diagnostic approach is possible by considering the clinical context and MRI data, where white matter abnormalities are observed predominantly in the parietal and occipital territories secondary to the formation of a vasogenic edema. The evolution is characterized by a reversibility of the lesions in case of early diagnosis and management.
