Strain characteristics of anterior cruciate ligament grafts inserted in the over-the-top technique were studied in an in-vitro model. A cruciate ligament attached at the center of the anatomical insertion sites provides a constant preload and accomplishes the principle of isometry during the whole range of motion. In contrast, the over-the-top reconstructed ligament shows an increasing laxity proportional to the degree of joint flexion due to an approach of the attachment areas. These observations indicate that a sufficient ligament function is not provided so that the over-the-top reconstruction cannot be recommended as method of choice.

Purpose: To compare side-to-side superior capsular reconstruction (SCR) with over-the-top SCR in terms of functional outcomes, pain relief and allograft survival rates.
Methods: Patients who had undergone arthroscopic dermal allograft SCR for massive irreparable rotator cuff tears and clinical follow-up for ≥2 years were recruited. All patients underwent postoperative assessment with routine radiographic analysis for acromiohumeral distances, ultrasound imaging 1 and 3 months after SCR and magnetic resonance imaging (MRI) at least 12 months after SCR.

Purpose: The Iliotibial band (ITB) is a fibrous thickening of the fascia lata originating at the iliac crest and inserting at Gerdy's tubercle on the lateral tibia. The ITB significantly contributes to lateral knee stabilisation.
