Pulmonary embolism (PE) is a potentially fatal condition with variable clinical presentations, ranging from classic respiratory symptoms to rare atypical manifestations. This report describes a 47-year-old woman who presented with acute, severe right upper quadrant abdominal pain, nausea, and vomiting without respiratory complaints. Initial investigations, including abdominal ultrasound and contrast-enhanced CT of the abdomen, revealed no intra-abdominal abnormalities.

Aims: Sudden arrhythmic death syndrome (SADS) refers to a sudden death, which remains unexplained despite comprehensive post-mortem examination and a toxicological screen. We aimed to investigate the impact of age and sex on the overall diagnostic yield and underlying aetiology in decedents with SADS using a combined approach of familial evaluation (FE) and molecular autopsy (MA).

Methods And Results: Consecutive referrals to a single centre for FE only, MA only or both, following a SADS death were included.

Efficacy of behavioral cough suppression therapy for refractory chronic cough or unexplained chronic cough: a meta-analysis of randomized controlled trials.

Ther Adv Respir Dis

December 2024

Department of Pulmonary and Critical Care Medicine, Tongji Hospital, School of Medicine, Tongji University, No. 389 Xincun Road, Shanghai 200065, China.

Background: The efficacy of behavioral cough suppression therapy (BCST) for refractory chronic cough (RCC) and unexplained chronic cough (UCC) remains unclear due to limited evidence from small-scale single-center studies.

Objective: To compile and assess the quality of evidence from randomized controlled trials to evaluate the effectiveness of BCST.

Design: This study included randomized controlled studies and self-controlled studies related to BCST involving adult patients with RCC or UCC.

Diagnostic performance of metagenomic next-generation sequencing based on alveolar lavage fluid in unexplained lung shadows.

Research Centre for Intelligent Healthcare, Faculty of Health and Life Sciences, Coventry University, Coventry, CV1 5FB, UK.

Background: Unexplained lung shadows are challenging in respiratory medicine, with both infectious and non-infectious etiologies. Lung biopsy is definitive but invasive, prompting a need for non-invasive alternatives. Metagenomic next-generation sequencing (mNGS) of bronchoalveolar lavage fluid (BALF) is emerging as a promising diagnostic tool.

X-linked lymphoproliferative disease (XLP) is a rare primary immunodeficiency with susceptibility and vulnerability to Epstein-Barr virus (EBV) infection. Most patients were diagnosed in early childhood and do not survive into adulthood. Here we reported an adult-onset XLP patient presenting with fever, dyspnea, and pulmonary nodules, mimicking respiratory infection at disease onset.
