This case highlights the critical role of early radiological screening by ultrasound in identifying uterine anomalies. In this report, we discuss a 39-year-old pregnant woman, gravida 4 para 3, and her fetus at gestational age 18 weeks. The patient was referred to the Obstetrics and Gynecology Emergency Department at Qatif Central Hospital, Saudi Arabia, from a private hospital due to an ultrasound study indicating a possible ectopic pregnancy with an abdominal fetal location.

Gastroschisis represents a congenital malformation characterized by the herniation of abdominal contents through a defect in the abdominal wall, predominantly situated to the right of the umbilical cord. The defect is characterized by the absence of a covering membrane, resulting in the free floating of extruded abdominal contents. Major complications associated with this condition include stillbirth, preterm delivery, and intrauterine growth restriction.

Threatened miscarriage is a common complication of early pregnancy characterized by symptoms of vaginal bleeding with/without abdominal cramps/pain in the first trimester. Progestogens are often administered for management of this condition. Presented herein is the protocol of an ongoing, multicentric clinical trial to investigate the efficacy and safety of micronized progesterone (natural progestogen) compared to dydrogesterone (synthetic isomer of progesterone).
