Introduction: The effectivity, evaluability of the colonoscopic procedure depends greatly on the preparation and the cleanliness of the colon. A large scale of laxatives used for colon preparation are also available in our country (phenolphtalein, Karlsbad-salt, saccharosum + sennosid-B solution, bisacodyl, powder mixtures).

Aims: The authors examined in 5 gastroenterology centres the tolerability and effectivity of two frequently used laxatives--saccharosum + sennosid-B solution and Na-picosulphate--during colonoscopic preparation in 157 patients.

Methods: Exclusion criteria were: severe anemia, renal insufficiency, cardiac failure, active ulcerative colitis and Crohn's disease, possible stenotising colonic process and hypersensibility to one of the compounds. The patients were randomised prospectively. In the case of the saccharosum + sennosid-B solution the colon preparation was performed according to the manufacturers prescriptions, in the case of the Na-pikosulfate the investigator's own procedure was used based on literature data. The tolerability of the preparation was assessed using a questionnaire. The investigators made their statements concerning the cleanliness of the different colon sections based on uniform criteria.

Results: Both methods showed good efficacy concerning the cleanliness of the colon. The patients considered the Na-picosulphate better tolerable--based on the questionnaire data. The authors consider the analysis of further laxatives to help improve the work of their fellow endoscopists.

Article Synopsis
  • Postcolonoscopy colorectal cancers often occur in the right colon due to missed adenomas and serrated polyps, and the study investigates whether water exchange (WE) improves detection rates compared to CO2 insufflation.
  • A total of 386 patients were randomly assigned to either WE or CO2 insufflation, with a reexamination by a blind endoscopist to determine the miss rate of adenomas and serrated polyps.
  • Results showed that WE significantly reduced the miss rates for serrated polyps and increased the detection of serrated polyps in the right colon, suggesting that WE is a promising alternative to traditional CO2 insufflation in colonoscopy.

Usefulness and safety of new ultrasmall-diameter colonoscope for cases with difficult insertion: a retrospective study.

Sci Rep

September 2024

Division of Gastroenterology and Hepatology, Department of Medicine, Nihon University School of Medicine, 30-1, Oyaguchi Kami-cho, Itabashi-ku, Tokyo, 173-8610, Japan.

Colonoscopies are widely available, but there are cases where insertion can be difficult, even for experienced endoscopists. EC-760XP/L, a new ultrasmall-diameter long scope, may be useful in such cases. This single-center retrospective study included 39 cases where colonoscope insertion was difficult even when previously conducted by an experienced endoscopist.

Performance of bowel preparation quality scales in patients with Crohn's disease.

Aliment Pharmacol Ther

October 2024

Department of Medicine, Division of Gastroenterology, Western University, London, Ontario, Canada.

Article Synopsis
  • The study aimed to evaluate various instruments for assessing bowel preparation (BP) quality in patients with Crohn's disease, a condition where BP performance is not well understood.!
  • Five different scales were used to analyze BP quality in videos from 40 patients, focusing on reliability and validity through statistical methods.!
  • Results showed that most instruments demonstrated substantial reliability, with a negative correlation between BP quality and disease activity scores, indicating these tools are useful in clinical practice and research for Crohn's disease patients.!

Background: Construct deep learning models for colonoscopy quality control using different architectures and explore their decision-making mechanisms.

Methods: A total of 4,189 colonoscopy images were collected from two medical centers, covering different levels of bowel cleanliness, the presence of polyps, and the cecum. Using these data, eight pre-trained models based on CNN and Transformer architectures underwent transfer learning and fine-tuning.

Colonoscopy is the standard procedure for screening, and surveillance of colorectal cancer, including the treatment for colonic lesions. Colonic spasm is an important problem from colonoscopy that affects both surgeons and patients. The spasm also might be the cause of longer cecal intubation time, difficulty of the procedure, and increased pain.
