The role of accidental hypothermia in drowning patients with out-of-hospital cardiac arrest: A nationwide registry-based cohort study.

Resuscitation

January 2025

Prehospital Center Region Zealand, Ringstedgade 61, 13th floor, 4700 Naestved, Denmark; Department of Clinical Medicine, University of Copenhagen, Blegdamsvej 9, 2100 Copenhagen, Denmark.

Aim: This study aimed to investigate the associations between hypothermia and mortality or poor neurological outcome in a nationwide cohort of drowning patients with out-of-hospital cardiac arrest (OHCA).

Methods: This nationwide, registry-based cohort study reported in-hospital data on drowning patients with OHCA following the Utstein Style For Drowning. Drowning patients with OHCA were identified in the Danish Cardiac Arrest Registry from 2016 to 2021.

Background And Importance: Prolonged emergency medical services' response times (EMS-RT) are associated with poorer outcomes in out-of-hospital cardiac arrest (OHCA). The patient access time interval (PATI), from vehicle stop until contact with patient, may be increased in areas with low socioeconomic status (SES).

Objectives: The objective of this study is to identify predictors of prolonged EMS-RT intervals, and to evaluate associations with clinical outcomes in OHCAs occurring in the largest metropolitan area in France.

Background: : Globally, there is significant variation in the out-of-hospital cardiac arrest (OHCA) survival rate. Early links in the chain of survival, including bystander cardiopulmonary resuscitation (CPR) and the use of an automated external defibrillator at the scene, are known to be of crucial importance, with strong evidence of increased survival rate with good neurological outcomes. The data from the Middle East are limited and report variable rates of bystander CPR and survival.

Article Synopsis
  • Cardiac arrest's varied causes make it hard to create precise treatment plans, but this study presents an intelligent system to improve in-hospital cardiac arrest (IHCA) management and increase CPR success rates.
  • The researchers used a hybrid method to generate virtual patient samples and employed a deep reinforcement learning module for better diagnosis and treatment, analyzing data from multiple hospitals in China.
  • Results showed the system outperformed others, especially when real data was lacking, achieving higher average reward scores that indicate improved efficacy in the treatment of IHCA.

Article Synopsis
  • This study aimed to validate prediction tools (CASPRI, GO-FAR, and PIHCA) for determining favorable neurological outcomes following in-hospital cardiac arrests by analyzing patient data from 2018 to 2023.
  • Out of 1,100 patients, only 42 (3.8%) had favorable outcomes; factors like age and respiratory failure were linked to better outcomes, with CASPRI showing the highest predictive ability.
  • The findings suggest that CASPRI and GO-FAR could be effectively integrated into clinical practices in Iran to improve treatment strategies for cardiac arrest patients.
