There are a variety of clinicopathologic situations of pituitary lesions that could cause a discordance of diagnosis between clinicians and pathologists. This may be caused by confusion of terminology such as "nonfunctioning adenoma" and "null cell adenoma." "Clinically nonfunctioning pituitary adenomas" comprise several pathologically different types of tumors that belong to one of three major cell lineages of adenohypophysial cell types. "Null cell adenoma" was originally defined by ultrastructural features but recently refers to immunonegative adenomas. Unique and unusual types of adenomas such as adenomas with "honeycomb Golgi" appearance and silent subtype 3 adenomas may cause a discordance of diagnosis. Because of mild elevation of prolactin levels, these adenomas are sometimes erroneously diagnosed as prolactinoma. Careful pathologic study with immunohistochemistry and electron microscopic examination, as well as communication between clinicians and pathologists, is vital. Confusion of terminology should be discussed. Awareness of rare disease entities is also required. Thorough analysis of individual cases with diagnostic inconsistency may provide a useful lesson for better understanding of endocrine diseases and for appropriate treatments.

Background: Exposure-related changes in exhaled nitric oxide (FeNO) and sputum eosinophils have not been thoroughly compared in the investigation of occupational asthma.
Objective: This study aimed at comparing the accuracies of the changes in FeNO concentrations and sputum eosinophil counts in identifying asthmatic reactions induced by occupational agents during specific inhalation challenges (SICs).
Methods: This retrospective multicenter study included 321 subjects who completed an assessment of FeNO and sputum eosinophils before and 24 h after SICs with various occupational agents, of whom 156 showed a positive result.

Purpose: Acromegaly patients occasionally achieve either of the remission criterion of IGF-1 or GH level postoperatively; however, treatment for patients with discordant IGF-1 and GH levels remains unclear. This study aimed to clarify the clinical courses and features of postoperative patients with normal IGF-1 and high GH levels and support their management.
Methods: Overall, 110 acromegaly patients underwent initial surgery and a 75-g oral glucose tolerance test (OGTT) 3 months postoperatively.

Summary Answer: It has been possible to obtain a karyotype comparable to preimplantation genetic testing for aneuploidy (PGT-A), in addition to a transcriptomic signature of embryos which might be suggestive of improved implantation capacity.
What Is Known Already: Conventional assessment of embryo competence, based on morphology and morphokinetic, lacks knowledge of molecular aspects and faces controversy in predicting ploidy status. Understanding the embryonic transcriptome is crucial, as gene expression influences development and implantation.
