A summary of the primary points follows: The microbiological quality of the dialysis fluid represents an independent determinant of the nutritional status, in addition to known factors such as dose of dialysis and biocompatibility of the dialyzer membrane. Ultrapure dialysis fluid adds to the cost of the dialytic treatment, but improves the nutritional status in long-term hemodialysis patients. Water quality needs to be routinely monitored to ensure that the potential risk to patients from chemical and bacterial contaminants is minimized; this requires the implementation of a quality approach. The layout and design of the water room and water system are key to producing and maintaining high-quality dialysis fluid. Routine monitoring and preventive maintenance of the water system will ensure that high-quality dialysis fluid is delivered.

Fluid overload trajectories and mortality in hemodialysis patients.

J Intern Med

December 2024

Fresenius Medical Care, Global Medical Office, Bad Homburg, Germany.

Background: Fluid overload remains critical in managing patients with end-stage kidney disease. However, there is limited empirical understanding of fluid overload's impact on mortality. This study analyzes fluid overload trajectories and their association with mortality in hemodialysis patients.

BACKGROUND Toluene poisoning can occur as a result of occupational exposure in industries such as painting, as well as through misuse, leading to complications such as neurological symptoms due to the accumulation of the metabolic byproduct of hippuric acid and metabolic acidosis. However, the exact mechanisms remain unclear. Hippuric acid is not removed by dialysis, so urinary excretion plays a central role.

Physiological and artificial solubilizing agents usually enhance apparent solubility of poorly soluble drugs, and in many cases also oral drug exposure. However, exposure may decrease in cases where micellization reduces the molecularly dissolved drug fraction, overriding the solubility advantage. While this information is critical to accurately anticipate the effect of drug micellization on oral absorption, the experimental determination of molecularly dissolved drug concentrations is complex and time consuming.

Peritoneal dialysis (PD) serves as a home-based kidney replacement therapy with increasing utilization across the globe. However, long-term use of high-glucose-based PD solution incites repeated peritoneal injury and inevitable peritoneal fibrosis, thus compromising treatment efficacy and resulting in ultrafiltration failure eventually. In the present study, we utilized human mesothelial MeT-5A cells for the in vitro experiments and a PD mouse model for in vivo validation to study the pathophysiological mechanisms underneath PD-associated peritoneal fibrosis.

Exploring Biomarkers for Excess Extracellular Fluid in the Context of Physical Function in Chronic Kidney Disease Patients.

J Pers Med

November 2024

Division of Nephrology, Department of Internal Medicine, CHA Bundang Medical Center, CHA University School of Medicine, Seongnam 13496, Republic of Korea.

: Fluid overload is an important risk factor for protein-energy wasting, which could lead to poor outcomes, such as higher morbidity and mortality, in patients with chronic kidney disease (CKD). This study aimed to validate the possible myokine as a biomarker of volume status in patients with non-dialysis CKD. : In total, 151 patients with CKD were enrolled from a single medical center.
