Purpose: To evaluate the use of preserved scleral graft with amniotic membrane transplantation (AMT) for the surgical repair of scleromalacia.

Methods: A prospective study of eight eyes (eight patients) was performed. We grafted glycerin-preserved sclera onto the areas of scleral thinning with impending perforation and then covered them with an amniotic membrane with a thick basement membrane instead of a conjunctival flap.

Results: All patients experienced loss of ocular pain and inflammation, rapid reepithelialization of the ocular surface, and marked improvement in visual acuity. All of the scleral grafts remained intact, and no recurrence of scleromalacia was observed.

Conclusion: Preserved scleral graft with AMT was simple, fast, and effective. It was particularly advantageous when large scleral defects or conjunctival scarring was present. With these therapeutic results, preserved scleral graft with AMT qualifies as a new treatment alternative for scleromalacia.

We report a case of acute angle closure secondary to pupillary block caused by a dislocated intraocular lens (IOL)-capsular tension ring (CTR) complex. A 68-year-old woman, who had undergone cataract surgery 28 months earlier, presented with acute right eye pain and blurred vision. Examination revealed elevated intraocular pressure (IOP) of 80 mmHg, corneal edema, and anterior chamber shallowing, with anterior displacement of the IOL-CTR complex observed on anterior-segment optical coherence tomography.

Minimal Access Lateral Approach in Lower Eyelid Blepharoplasty for Three-Dimensional Recontouring.

Lower eyelid blepharoplasty has historically been approached from one of only two techniques, either the anterior subciliary incision or from the posterior transconjunctival approach. Both have been used with subtractive techniques of skin and fat resection in most cases and both have had issues with post operative lower lid retraction with scleral show, albeit less with the posterior approach. Lateral acces recontouring does not transgress either the anterior or posterior functional muscles of the lower lid and avoids damaging the innervation of the lower lid orbicularis.

School of Medicine, Iran University of Medical Sciences, Hemmat Expressway, Tehran, Iran.

This article's raison d'etre is simple: to deliver a decidedly lucid and sanguinely compelling description of a lower eyelid surgical rejuvenation strategy. By bestowing the surgeon greater control of the lower eyelid, a more esthetic result with fewer complications can be reliably delivered. The pinch blepharoplasty does so by offering 3 unique advantages: First, it respects and preserves both esthetic and functional eyelid posture.
