Partial necrosis of cervix uteri as a complication to HELLP-syndrome.

Background: The HELLP-syndrome is a rare complication to pregnancy and a potential fatal condition.

Case: We report such a case in a 32-year-old para 2, gravida 1 woman with no known risk factors. A lower transverse uterotomy was performed at 29 weeks and 6 days' gestation based on an indication of HELLP-syndrome. Uncharacteristically lower back pain developed on the second day postoperatively and a gynaecological examination was performed. A partial necrotic cervix uteri was found with many trombosed vessels and necrotic tissue.

Conclusion: Partial necrosis of cervix uteri can be seen as a complication to the HELLP-syndrome.

The injectable contraceptive, depot medroxyprogesterone acetate (DMPA), is associated with compromised cervical mucosal barriers. High-resolution spatial transcriptomics is applied here to reveal the spatial localization of these altered molecular markers. Ectocervical tissue samples from Kenyan sex workers using DMPA, or non-hormonal contraceptives, underwent spatial transcriptomics and gene set enrichment analyses.

Aim: Our study aimed to evaluate the effectiveness of ultrasound-indicated cerclage in singleton pregnancies with cervical shortening (<25 mm), excluding those with a history of preterm birth (PTB) and infection/inflammation.

Materials And Methods: Among the 1556 women admitted for a cervix measuring <25 mm via transvaginal ultrasound at Juntendo University Urayasu Hospital between January 2001 and December 2023, our study focused on 47 singleton patients with no prior history of PTB. After receiving information on both risks and benefits, 25 patients opted for ultrasound-indicated cerclage (cerclage group), while 22 chose expectant management (expectant management group).

The presence of high-risk human papillomavirus (HR-HPV) contributes to the development of cervical lesions and cervical cancer. Recent studies suggest that an imbalance in the cervicovaginal microbiota might be a factor in the persistence of HR-HPV infections.
