Acute vertebrobasilar dissection may cause subarachnoid hemorrhage by rupturing through the adventia or cerebral infarct by progressive occlusion of the true lumen. Recent reports on the endovascular management of this condition have focused on treatment of pseudoaneurysms. We report two cases where angioplasty or stent placement was successfully used to improve compromised blood flow secondary to vertebrobasilar dissection.

Bilateral vertebral artery dissections account for only 8% of all vertebral artery dissections and cause just 2% of all ischemic strokes. They can occur spontaneously, even without any triggering factor. Classical clinical findings, such as headache or neck pain, may be absent, particularly in the context of a stroke.

Purpose: Arterial fenestration involves an artery emerging from the same embryological origin, with its lumen divided into two channels that subsequently rejoin. This kind of vascular variant has consequences, such as focal defects in the histologic layers of the arteries, hemodynamic stress, and challenges for endovascular procedures in the management of vascular pathologies.
