We describe a woman with metastatic breast cancer treated with multiple rounds of prior cytotoxic and endocrine therapies, who presented with ipsilateral diaphragmatic paralysis. The use of anastrozole, a highly selective nonsteroidal aromatase inhibitor (1 mg daily), successfully induced remission of metastatic tumors in our patient, who is partially recovering from diaphragmatic paralysis.

Purpose: The suprascapular nerve is situated between the prevertebral fascia and the superficial layer of deep cervical fascia and on the surface of the middle and posterior scalene muscles before it reaches the suprascapular notch. Consequently, we hypothesized that injecting local anesthetics (LAs) there would introduce a new block approach for blocking the suprascapular nerve, ie, extra-prevertebral fascial block. This report details a case of acute idiopathic velopharyngeal insufficiency in a previously healthy eight-year-old girl, presenting with sudden voice alteration and nasal regurgitation following mild respiratory symptoms. Physical examination identified unilateral velar paralysis with open rhinolalia, without additional neurological deficits. Extensive diagnostic evaluation, including nasopharyngoscopy, cerebral and cervical imaging, and infectious serologies, yielded unremarkable findings.
