34-years old patient with suspected massive pulmonary embolism (PE) is described. D-dimer test, venous ultrasonography and spiral-CT were used to rule out suspected PE. Bilateral pneumonia was diagnosed. Difficulties in the differential diagnosis of pneumonia and PE are discussed.
