Objectives: To report the case of a newborn with a supernumerary penis. To make this extremely rare pathological entity known, although the newborn was not operated due to religious beliefs, so that histological evaluation was not performed.

Methods: We report the case of a newborn that was found to have a perianal malformation at the time of birth. On call, the urologist diagnosed all the characteristics of a supernumerary penis with a blind urethral meatus and empty scrotum after physical examination.

Results/conclusion: With the diagnosis of supernumerary penis on physical examination proper tests, like blood tests, X-rays and sexual chromosomes were performed, but surgery was not allowed by his parents and the patient was discharge from the hospital.

The idiopathic hypogonadotropic hypogonadism (IHH) is portrayed as missing or fragmented pubescence, cryptorchidism, small penis, and infertility. Clinically it is characterized by the low level of sex steroids and gonadotropins, normal radiographic findings of the hypothalamic-pituitary areas, and normal baseline and reserve testing of the rest of the hypothalamic-pituitary axes. Delay puberty and infertility result from an abnormal pattern of episodic GnRH secretion.
