Heart failure is a growing public health problem owing to his increase in incidence and prevalence, and his substantial morbidity and mortality. It consumes a significant proportion of the health care budget in most developed countries. A multidisciplinary management program can substantially reduce the risk of readmission improving the living standard. It requires several health professionals: the hospital discharge summary can constitute an efficacious medium for the communication between them and with the patient, provided that it includes information about life-style changes, etiology, co-morbidity, prognosis and therapy.
