Autonomic Component of Carpal Tunnel Syndrome.

J Hand Surg Am

January 2025

Musculoskeletal Translational Innovation Initiative, Carl J. Shapiro Department of Orthopaedic Surgery, Beth Israel Deaconess Medical Center, Harvard Medical School, Boston, MA.

Carpal tunnel syndrome (CTS) is the most common compression neuropathy. The median nerve contains sensory, motor, and sympathetic fibers. Involvement of the different fibers of the median nerve in CTS may vary; hence, one of the sensory, motor, or autonomic dysfunctions may be dominant.

Flow-mediated dilation decreases in women, but not men, following mild acute intermittent hypoxia.

J Appl Physiol (1985)

January 2025

Department of Kinesiology, School of Public Health, University of Maryland, College Park, Maryland, United States.

Acute, intermittent exposure to mild hypoxia increases sympathetic nervous system activity, decreases arterial shear rate, and increases blood pressure (BP) in young men. Given that the BP-raising effects of intermittent hypoxia (IH) are less evident in young women, we sought to examine the influence of sex on macro- and microvascular function following IH. BP, macrovascular function [brachial artery flow-mediated dilation (FMD)], microvascular function [reactive hyperemia area under the curve (RHAUC)], and brachial artery shear rate [shear rate area under the curve (SRAUC)] were measured before and following 30 min of IH in 18 young adults (8 women, 23 ± 5 yr).

B-type natriuretic peptides (BNP) are produced and secreted by the myocardium to reduce blood pressure and cardiac load. They cause vasodilation, natriuresis, growth suppression, and inhibition of the sympathetic nervous system and the renin-angiotensin-aldosterone system. The measurement of plasma BNP levels provides clinically useful information concerning the diagnosis and management of left ventricular dysfunction and heart failure, complementing other diagnostic testing procedures.

Improvement of Post-sympathectomy Raynaud's Syndrome With Spinal Cord Stimulation.

Cureus

October 2024

Neurosurgery, The Second Affiliated Hospital of Zhejiang University, School of Medicine, Hangzhou Zhejiang, CHN.

Raynaud's phenomenon is a vascular disorder, characterized by vasospasm-induced discoloration, numbness, and pain in the extremities. While pharmacological treatments and sympathectomy are commonly employed, many patients experience symptom recurrence, and effective therapies for refractory cases remain limited. This case study presents a 60-year-old male with severe Raynaud's symptoms, including fingertip necrosis, unresponsive to pharmacotherapy and endoscopic thoracic sympathectomy.
