Disseminated nocardiosis in an immunocompetent child. | LitMetric

We report a 2-month-old child with a disseminated Nocardia farcinica infection that presented with suppurative lymphatic abscess. The child did not have any predisposing factors and responded to treatment with co-trimoxazole and amikacin. This is first case report of disseminated nocardiosis caused by Nocardia farcinica in an immunocompetent child.

Severe pneumonia caused by in a patient with bronchiectasis and IgA nephropathy: a case report.

Front Med (Lausanne)

February 2025

Department of Nephrology, Yueyang Hospital of Integrated Traditional Chinese Medicine and Western Medicine, Shanghai University of Traditional Chinese Medicine, Shanghai, China.

Background: species are rare opportunistic pathogens in the clinic, with strong invasiveness and dissemination, that can cause serious pulmonary infection, especially in immunocompromised patients, chronic lung diseases and hormone use, and is easy to be missed and misdiagnosed, preventing patients from obtaining timely and effective treatment, resulting in a high mortality rate.

Case Presentation: Here, we present a rare case of a patient with chronic bronchiectasis and IgA nephropathy who developed pneumonia shortly after hormone therapy. The patient presented with tongue and lip ulcers, chest distress, cough, expectoration, and fever as the initial symptoms, which were extremely similar to common bacterial pulmonary infections.

Nocardiosis caused by is exceptionally rare and characterized by a high mortality rate. It typically affects immunocompromised patients, resulting in severe pulmonary or disseminated infections, and is notorious for abscess formation. Empyema resulting from nocardiosis is even less common.

We report a 73-year-old immunocompromised man with multiple muscle abscesses, in a toxic state with neurological deficits, with a history of panhypopituitarism, diabetes, hypothyroidism, and recent SARS-CoV-2 infection. MRI showed abscesses in the right chest wall, right thigh (peri-implant), and left adductor. Elevated inflammatory markers, comorbidities and clinical presentation suggested disseminated fungal or Nocardial infection.

Nocardia species are aerobic, Gram-positive actinomycetes often causing opportunistic infections, particularly in immunocompromised hosts. Nocardia farcinica is notable for disseminated nocardiosis (DN), frequently involving the lungs and central nervous system (CNS). Cushing's syndrome (CS), characterized by hypercortisolemia, further predisposes patients to severe infections.
