Aim: To report present state of iatrogenic drug-induced esophageal injury (DIEI) induced by medications in a private clinic.
Methods: Iatrogenic drug-induced esophageal injury (DIEI) induced by medications has been more frequently reported. In a private clinic we encountered 36 cases of esophageal ulcerations complicating doxycycline therapy in a mainly younger Saudi population (median age 29 years).
Results: The most frequent presenting symptoms were odynophagia, retrosternal burning pain and dysphagia (94 %, 75 % and 56 %, respectively). The diagnosis was according to medical history and confirmed by endoscopy in all patients. Beside withdrawal of doxycycline, when feasible, all patients were treated with a proton-pump inhibitor (PPI) and a prokinetic. Thirty patients who reported to the clinic after treatment were improved within 1-7 (median 1.7) days.
Conclusion: Esophageal ulceration has to be suspected in younger patients with odynophagia, retrosternal burning pain and/or dysphagia during the treatment with doxycycline.

Background: Esophageal ulcers can arise not only from malignant lesions but also from benign diseases, such as tuberculosis. These ulcers may mimic the radiological features of esophageal malignancy or tuberculosis on PET/CT, leading to diagnostic challenges.

Cytomegalovirus (CMV) and Epstein-Barr virus (EBV) are important causes of viral esophagitis mainly in immunocompromised individuals. Both viruses lead to development of focal ulcerations in the esophagus. While there have been rare case reports of esophagitis in immunocompetent individuals, there has not been a single reported case of coinfection with both CMV and EBV in an immunocompetent individual and presenting with diffuse esophageal ulceration.

Introduction: Roux-en-Y gastric bypass (RYGB) reversal might be necessary to alleviate refractory surgical or nutritional complications, such as postprandial hypoglycemia, malnutrition, marginal ulceration, malabsorption, chronic diarrhea, nausea and vomiting, gastro-esophageal reflux disease, chronic pain, or excessive weight loss. The surgical technique of RYGB reversal is not standardized; potential strategies include the following: (1) gastro-gastrostomy: hand-sewn technique, linear stapler, circular stapler; (2) handling of the Roux limb: reconnection or resection (if remaining intestinal length ≥ 4 m).
Case Presentation: We demonstrate the surgical technique of a laparoscopic reversal of RYGB with hand-sewn gastro-gastrostomy and resection of the alimentary limb with the aim of improving the patient's quality of life.
