The first case of a ventrally exophytic pontine glioma with extensive and dense calcification is reported. A 10-year-old boy presented with a generalized seizure. The skull radiogram and CT scan showed a dense area of calcification, revealed by MRI as the core of a ventrally exophytic tumor. The pathologic diagnosis was fibrillary astrocytoma.

Intramedullary spinal cord metastasis (ISCM) is a rare manifestation of renal cell carcinoma (RCC). A 73-year-old man presented with left shoulder pain and left upper extremity weakness for two months. Magnetic resonance imaging (MRI) revealed intramedullary and intradural extramedullary lesions at the C5 level, compressing the spinal cord from the center of the cord and the left ventral side.

Background: Minor salivary glands can be found in the ventral and anterior part of the tongue; these glands can rarely develop mucoceles that, due to their rarity and their unusual clinical appearance, may present an interesting differential diagnosis. Mucoceles appear as an exophytic, sometimes pedunculated, lesion, which is a feature that is due to the absence of a capsule; thus, the glands are right beneath the mucosa and over the muscle tissue. The aim of this article is to retrospectively present and discuss the anatomy, pathology, clinical features and therapy of several cases of Blandin-Nunh mucoceles collected from two different institutions.

Background And Importance: Ventral midbrain glioma is an extremely rare lesion in adults. The endoscopic endonasal approach is potentially a better alternative to transcranial approach for this challenging lesion and has not been described in the literature.
Clinical Presentation: A 22-yr-old woman previously operated through an interhemispheric approach for ventral midbrain glioma with extension in the suprasellar cistern presented with severe headache and diplopia.

Background: Brainstem glioma is the leading cause of morbidity and mortality among all central nervous system tumors, especially in childhood as it represents about 20% of all pediatric brain tumors. Therefore, this study aimed to present our experience in a tertiary center in a developing country with limited resources for the surgical management of exophytic brainstem gliomas.
Methods: This retrospective study included pediatric patients with brainstem (midbrain, pontine, or medullary) focal or diffuse gliomas whether low or high grade that had dorsal, ventral, or lateral exophytic component who were presented to our hospitals from January 2019 to January 2021.

Surgical treatment of brainstem cavernoma is controversial. With modern surgical technique, safe and complete removal of selected brainstem cavernoma is possible. The choice of optimal corridor must consider various factors, including eloquent structures of the brainstem, cavernoma shape and location, as well as the presence of an exophytic portion to the lesion.
