Subglottic laryngeal paraganglioma.

Laryngoscope

Department of Otolaryngology-Head and Neck Surgery, University of Minnesota Medical School and Hennepin County Medical Center, 701 Park Avenue, Minneapolis, MN 55415, USA.

Published: March 2003

Objectives: To review the clinical and pathological picture of laryngeal paragangliomas and compare laryngeal paragangliomas with the three other types of laryngeal neuroendocrine neoplasms.

Study Design: The study presents a case review of a patient with a subglottic laryngeal paraganglioma treated at our institution, with a literature review of the laryngeal neuroendocrine neoplasms. Included is a review of all four neuroendocrine neoplasms, taking a close look at a comparison between laryngeal paragangliomas and atypical carcinoids tumors. All cases of subglottic laryngeal paragangliomas reported in the literature were also compiled and tabulated.

Methods: The patient's record was reviewed for age, symptomatology, workup, and surgical procedures performed.

Results: The patient was disease free 36 months after undergoing a tracheostomy for airway control, followed by a cricoid split for complete excision of the tumor.

Conclusions: It is important to differentiate among the four laryngeal neuroendocrine neoplasms, especially between atypical carcinoids and paragangliomas. Each tumor requires different treatments, with the former requiring a more aggressive approach. Paragangliomas must be completely excised to prevent their recurrence, and this approach should be considered a curative treatment. In addition, the immunohistochemical battery is of paramount importance in obtaining the correct pathological diagnosis.

This report describes a rare case of a paraganglioma occurring beneath the vocal folds. During the preoperative biopsy, we encountered significant hemorrhage, forcing us to stop the procedure and preventing us from obtaining a definitive diagnosis. Despite these challenges, the eventual surgery had a good outcome.

Article Synopsis
  • The study focused on the surgical outcomes and complications of carotid body paragangliomas in 21 dogs.
  • The complication rate during surgery was high at 52%, with issues like airway obstruction and aspiration pneumonia being common.
  • Despite some complications, surgical removal had low mortality (4.7%) and median survival of 554 days, with one- and two-year survival rates of 61% and 42%, making it a significant report in veterinary literature.

Article Synopsis
  • Neuroendocrine neoplasms (NENs) are uncommon in the head and neck, with laryngeal neuroendocrine carcinoma (NEC) being the most frequent, primarily affecting older men who smoke.
  • Approximately 700 cases have been documented, and the incidence rate is around 0.23%, leading to challenges in diagnosis and treatment due to their vague symptoms.
  • Treatment often combines surgery, chemotherapy, and radiotherapy, as illustrated by a case study of a 61-year-old female nonsmoker who received endoscopic excision and follow-up radiotherapy for supraglottic laryngeal NEC.
