Neurosarcoidosis (NS) is a rare form of sarcoidosis, with isolated hypothalamic-pituitary involvement being exceptionally uncommon. We report a 20-year-old woman presenting with polyuria, galactorrhea, amenorrhea, and substantial weight loss. Hormonal evaluation revealed hypopituitarism with arginine-vasopressin deficiency and hyperprolactinemia.

Background: Rathke cleft cysts (RCCs) are benign sellar/suprasellar lesions that result from mucin-secreting vestigial remnants within the pars intermedia of the pituitary gland. When symptomatic, they can present with retro-orbital headaches, visual field defects, and/or pituitary dysfunction.
Observations: A 35-year-old female presented with subacute retro-orbital headache, right ptosis, and blurred vision.

Survivors of sellar/suprasellar tumors involving hypothalamic structures face a risk of impaired quality of life, including tumor- and/or treatment-related hypothalamic obesity (TTR-HO) defined as abnormal weight gain resulting in severe persistent obesity due to physical, tumor- and/or treatment related damage of the hypothalamus. We analyze German claims data to better understand treatment pathways for patients living TTR-HO during the two years following the index surgical treatment. Germ cell tumors (GCTs) commonly develop in the pineal and suprasellar regions, with the most common GCTs being germinomas. In this report, a 22-year-old male presented with progressive right-sided weakness, and his imaging was consistent with a left thalamic high-grade glioma. A stereotactic biopsy was performed, revealing a germinoma, but radiation therapy was not done, and the patient was lost to follow-up.
