Spasticity is common in patients with a variety of central nervous system disorders. It can lead to significant disability or cause complications that may result in severe morbidity. In such patients, treatment of spasticity is warranted. Several oral and parenteral medications are available for use in the treatment of spasticity. This article reviews the pharmacological properties and therapeutic effectiveness of these medications to provide a practical objective guide for physicians who may be involved in the management of spasticity.

Botulinum toxin type A is a first line choice in the treatment of spastic muscle overactivity. However, targeting the muscles involved in the deformity with the appropriate dose as well as choosing the goal to achieve and predicting the expected results can be challenging. Diagnostic nerve block with anaesthetics rapidly and temporarily suppresses overactivity of the selected muscle allowing clinicians to identify the involved muscles and the potential improvement of botulinum toxin injections.

This work studies upper-limb impairment resulting from stroke or traumatic brain injury and presents a simple technological solution for a subset of patients: a soft, active stretching aid for at-home use. To better understand the issues associated with existing associated rehabilitation devices, customer discovery conversations were conducted with 153 people in the healthcare ecosystem (60 patients, 30 caregivers, and 63 medical providers). These patients fell into two populations: spastic (stiff, clenched hands) and flaccid (limp hands).
