We describe a patient with a giant ruptured sinus of Valsalva aneurysm causing a massive left-to-right shunt. The diagnosis was made by transoesophageal echocardiography and confirmed by angiography. We outline briefly a diagnostic and therapeutic work-up.

Introduction: Polycystic liver disease and giant hepatic hemangioma may present with severe symptom burden and indicate orthotopic liver transplantation. The left-to-right piggyback approach is a useful technique for performing total hepatectomy of enlarged livers.
Objective: The purpose of this study is to analyze the results of liver transplantation in patients with benign massive hepatomegaly.

Anomalous systemic arterial supply to the basal segment of the lung (ABLL) is a rare cause of haemoptysis. ABLL may be complicated by massive haemoptysis, heart failure due to left-to-right shunt and infection. We describe a case of this condition presenting in the third trimester of pregnancy.

A man in his 40s presented with haematemesis and melaena and was found to have a massive variceal bleed. Endoscopic procedures were ineffective at controlling the bleed; thus, an emergent transjugular intrahepatic portosystemic shunt (TIPS) procedure was performed. There were no noted complications from the procedure and the patient was eventually discharged home.
