Factors affecting outcome of colposuspension.

J Obstet Gynaecol

Urogynaecology Unit, Department of Obstetrics and Gynaecology, Leisceter General Hospital, Gwendolen Road, Leicester LE5 4WP, UK.

Published: January 2002

A prospective follow-up study of 196 women who underwent a colposuspension operation for genuine stress incontinence (GSI) at the Leicester General Hospital between June 1991 and May 1996 (inclusive) was carried out to review the outcome of surgery. The outcome was based on clinical symptomatology, demonstration of stress incontinence in the clinic and limited postoperative urodynamic studies. The clinical cure rate was 91.33%. Continence rate was reduced by age above 70 years and post-operative urinary tract infections, and not affected by previous colposuspension, or anterior colporrhaphy. On the other hand, previous colposuspension, previous pelvic floor operations and urinary tract infections caused a significantly higher incidence of voiding problems. Older women and those who have had previous bladder neck surgery need special attention in assessment, counselling and in choice of surgery. Reducing postoperative urinary tract infections might help improve the overall outcome of the operation.

Citrate in autosomal dominant polycystic kidney disease: biomarker or therapeutic agent?

Curr Opin Nephrol Hypertens

March 2025

Nephrology Division, Universidade Federal de São Paulo (UNIFESP), São Paulo, Brazil.

Purpose Of Review: This review highlights the latest findings regarding hypocitraturia in autosomal dominant polycystic kidney disease (ADPKD), from both experimental and clinical studies, exploring the underlying pathophysiology and potential therapeutic approach.

Recent Findings: Experimental studies have shown that the lodging of microcrystals in the tubules can trigger cyst formation and growth in polycystic kidney disease (PKD). ADPKD patients are prone to developing hypocitraturia in early stages, which could predispose to calcium microcrystal formation.

This case report describes a 70-year-old male presenting with limb weakness, urinary retention and tandem cervical and lumbar spinal stenosis with complicating white cord syndrome, a rare reperfusion injury post decompression surgery. Initially admitted following an unwitnessed fall, the patient's neurological examination indicated that progressive weakness of the limbs and sensory loss etiology is cervical and lumbar spondylosis with severe spinal canal stenosis, confirmed by imaging. Due to rapid deterioration, he underwent C5 corpectomy, cervical decompression and fusion.

Objectives: To determine the health utility values (HUVs) of overactive bladder (OAB), defined as urinary urgency, usually accompanied by urinary daytime or nocturnal frequency, with or without urinary incontinence, among adults aged ≥65 years and to assess the HUV decrements (disutilities) of OAB according to its severity.

Methods: This cross-sectional Internet-based study was conducted between 2 and 9 November 2023, with quota sampling with equal probability for each sex and age group (age 65-74 years and ≥75 years). OAB was defined as an urgency score of ≥2 points and a total score of ≥3 points based on the Overactive Bladder Symptom Score.

Objectives: To analyse and compare the functionality of extraluminal and intraluminal artificial urinary sphincters (AUSs), an in silico procedure has been defined and applied. Design and reliability assessments of the AUS are typically performed using a clinical approach, which does not provide data on mechanical stimulation of urethral tissues. Mechanical stimulation may determine tissue degeneration, such as urethral atrophy or erosion, the main causes of AUS failure.

Objective: To evaluate the correlation between ATP-binding cassette (ABC) transporter expression and therapeutic efficacy of enfortumab vedotin (EV), an antibody-drug conjugate targeting Nectin-4, in urothelial cancer, as only a few studies have been conducted on this topic.

Patients And Methods: This retrospective study included 20 patients with metastatic urothelial carcinoma (mUC), including bladder and upper urinary tract cancers, who were treated with EV at Dokkyo Medical University Hospital between 2022 and 2024. Immunohistochemical staining was performed on formalin-fixed, paraffin-embedded tissue samples.
