The commisures are the two extremities of the mouth. They make a big complex in their architecture and their function. Their repairing should have an important attention to their final shape and function. The frequency of the retraction of the scarring tissue imposes additional muscular exercise which has to be premature, intense, and prolonged.

: Surgical treatment of extensive tumors of the lower lip generates important defects in its structure and functions. Over time, a multitude of reconstruction options for these defects have been imagined. Unfortunately, the majority involve the use of both local and regional flaps, which either lack labial structure or disorganize the oral commissures as nodal oral function points.

Lip ulcers associated with endotracheal tube fixation are a known complication in adults, but their prevalence in neonates and preterm infants remains unclear. We report a case of a right oral commissure ulcer that developed during endotracheal tube fixation at the right oral commissure and left lateral decubitus positioning in an extremely preterm infant with unilateral pulmonary interstitial emphysema (PIE). A male infant was born at 24 weeks and four days of gestation, weighing 696 gm.
