A case report of a syphilitic aortic aneurysm in a 37-year-old male, presenting as upper abdominal pain accompanied by a pulsatile abdominal mass, is discussed.

We describe a patient with an asymptomatic infrarenal abdominal aortic aneurysm. Treatment decisions were complicated by the presence of a left congenital pelvic kidney supplied by two renal arteries originating from the proximal common iliac arteries bilaterally and respiratory status that was prohibitive to open repair. A hybrid surgical repair was performed with a bifurcated aortic endograft and parallel grafting to revascularize the pelvic renal arteries.

Despite advancements in surgical techniques and critical care, managing complications of type A and B aortic dissections remains challenging. Common morbidities include paraplegia, renal failure, stroke, and intestinal ischemia. Risks are especially high in extensive repairs, such as Crawford extent II thoracoabdominal aortic aneurysms, and in older patients or those with heart failure, poor pulmonary function, or renal disease.

Background And Aim: Our previous research indicates that sarcoplasmic/endoplasmic reticulum calcium ATPase 2 (SERCA2) dysfunction facilitates the phenotypic transformation of aortic smooth muscle cells (ASMCs) and intensifies aortic aneurysm through the regulation of calcium-dependent pathways and endoplasmic reticulum stress. Our hypothesis is that additional mechanisms are involved in aortic aneurysm and atherosclerosis induced by SERCA2 dysfunction from the perspective of ASMC phenotypic transformation.
Methods & Results: In SERCA2 dysfunctional mice and their control littermates, ASMCs were isolated to analyze protein expression and cell functions, and angiotensin II was infused into these mice that were backcrossed into LDL receptor deficient background to induce aortic aneurysm and atherosclerosis.

Objective: Cloud-based, surgical augmented intelligence (Cydar Medical, Cambridge, UK) can be used for surgical planning and intraoperative imaging guidance during complex endovascular aortic procedures. We aim to evaluate radiation exposure, operative safety metrics, and post-operative renal outcomes following implementation of Cydar imaging guidance using a manually matched cohort of aortic procedures.
Methods: We retrospectively reviewed our prospectively maintained database of endovascular aortic cases.
